Just took one for ya Dean! This is the patch I added to my mask a little while ago. All I had to go on was a couple freeze frames of the DVD and a small shot of one Cropsy had done a long time ago, so I had to estimate the exact sizing. Not positive how accurate it is, but looked pretty good to me!
The size of it is 3\" wide and 2\" high, round the corners, and try to get it as symetrical as possible. I made 2 or 3 templates, and went through a couple pieces of leather before I got it right.
